Multiattack. The monstrous dog makes two attacks: one with its bite and one with its tail. It can’t make both attacks against the same target.
Bite. Melee Weapon Attack: +10 to hit ( 1d20+10 ), reach 10 ft., one target. Hit: 33 4d12+7 piercing damage plus 7 ( 2d6 ) necrotic damage and heals for the same amount of necrotic damage dealt. If the target is a Large or smaller creature, it is Grappled (escape DC 17). Until this grapple ends, the target is Restrained, and the monstrous dog can’t bite another target. However, at the start of the dog's turn the grappled creature automatically takes 33 piercing and 7 necrotic damage from being chewed on.
Tail. Melee Weapon Attack: +10 to hit ( 1d20+10 ), reach 10 ft., one target. Hit: 20 3d8+7 bludgeoning damage and the target is knocked up in the air and away from the monstrous dog 1d6*10 feet and knocked prone. Any creature hit in this way takes falling damage.
For every four opponents the Alpha faces in combat it gains 1 legendary action. It may use either its bite, tail or charge attack.
When encountered, roll the timer (1d4). Each time the timer drops to 1 on the monstrous dog's turn roll the timer again, this is the number of other monstrous dogs that show up as well as how long before more turn up.
