Symbiosis. When the Synaptic Leech has a humanoid creature grappled, that humanoid gains +2 to all stats (to a maxiumim of 20) and advantage on wisdom saves. The grappled creature also gains an additional 5ft of movement and can commune with the Leech telepathically. If the Synaptic Leech dies while connected to a creature, the grappled creature must make a DC 18 Wisdom save or take 4d4 Psychic damage, or half as much on a success. Fail or Success, the grappled creature gains 2 levels of exhaustion.
Pounce and Engage Symbiosis. The jumps up to 30ft. toward a creature it can see, and attempts to connect itself to it, the targetted creature must make a DC 14 Strength Save or be forced to make a DC 16 Wisdom Save or be grappled by the Leech. The first turn a creature is grappled, they suffer 10 (2d6+3) Piercing damage. Disconnect. The Synaptic Leech releases its grapple from the creature. The grappled creature when released takes 14 (3d6+3) Psychic damage.
Reduce Burden (1/Long Rest). The Leech reduces the number of damage a creature it is grappling takes by 2d10.
The Leech usually waits somewhere with access to jump but also space to hide, when in combat, the Leech will remain still in an effort to confuse potential predators.
Synaptic Leeches are incredibly rare, and most often found deep underground. On the surface, they are found attempting to hide in piles of bones.
